Notice
If you have received a judgment against
someone and wish our office to levy on
a person's bank account, please submit
two (2) completed forms with your writ
of execution. We will serve them at the
financial institution and the bank will
retain one completed form while freezing
money in the debtor's bank account and
forward the other completed form to the
judgment debtor. |

to Levy on Earnings
If you have received a judgment against
someone and wish our office to levy on
a person's wages, please mail two (2)
completed forms to the judgment debtor
ten days before bringing the writ of execution
to the sheriff's office. If the debtor
has any exemptions, the debtor should
send one form to his/her employer and
one back to the creditor. |

This form is to be dated and signed
when you have mailed the Execution Exemption
Notice and Notice of Intent to Levy on
Earnings to the judgment debtor (see above).
This form needs to be submitted with the
writ of execution to the sheriff's office.
