To apply for a passport, you must furnish:

COMPLETE APPLICATION FORM - complete all required sections; do not sign the application until you are in the presence of the person who will execute the application.
For passport information check here (http://travel.state.gov) The required forms are also available at the Otter Tail County Recorder's Office.

CERTIFIED COPY OF YOUR BIRTH CERTIFICATE - a hospital copy is not acceptable for this purpose. Obtain a certified copy from the County of State in which you were born. If you have previously been issued a passport, you must submit the most recent passport rather than a birth certificate except, if the child is under age 14 a birth certificate must be used.

2 PHOTOGRAPHS - must be identical and must meet proper size and mounting temperature requirements.

$67.00 FEE - check or money order made payable to U.S. Department of State. Age 16 and under $62.00.

$30.00 FEE - fee for the execution of your passport, payable to County Recorder.

MUST HAVE A VALID PICTURE ID FOR PURPOSES OF IDENTIFICATION

ALLOW 4 TO 6 WEEKS FOR THE PASSPORT TO BE ISSUED TO YOU.
