PREVENTS EPIDEMICS AND THE SPREAD OF DISEASE
Disease Prevention:
Preventing the spread of epidemics and the spread of disease. Mantoux testing and medication management related to tuberculosis.

Animal Bite Follow Up:
Contacting the animal owner and victim to determine if their has been an exposure to rabies virus.

Immunizations:
Clinics at schools, worksites, community events, and public clinics to prevent the spread of vaccine preventable disease. See 'Immunizations' for more information.

Refugee Health:
Visits to refugees to link them to resources needed to address unmet health needs, teaching about the health delivery system in this country and case management as needed to address health issues such as completion of the recommended immunizations.

Information about indoor air quality issues and concern.

Lead Education:
Health teaching to families with children with a high blood lead level and to the public regarding the prevention of lead poisoning.

Lead Poisoning Prevention:

Lead Poisoning Prevention.

Public Health Hazards:
Assessments for the detection and abatement of health hazards that affect the public.
Methamphetamine lab cleanup guidelines.

Water Quality:
Education regarding protecting or disinfecting private drinking water.

PREVENTS INJURIES
Injury Prevention:
Working with other agencies to prevent child abuse and neglect. Health teaching related to car seat safety and home safety checklist. See also: www.buckleupkids.state.mn.us


PROMOTES AND ENCOURAGES HEALTHY BEHAVIORS
Caring Connections :
Health teaching related to parenting and child growth and development. Health teaching to pregnant teens to obtain prenatal care, and preparation for childbirth and to teen parents for child growth, development and safety.

Nurse Family Partnerships:(nursefamilypartnership.org)
First time pregnant women to prepare for the birth and caring for infant and toddlers to age 2.

Child and Teen Checkup/Early Childhood Screening:
Clinics for a child and teen checkup for children age 3 ½ to 5 as a part of early childhood screening. Clinics for the early childhood screening required for children age 3 ½ to 5 prior to starting kindergarten. See Child & Teen for more information.

WIC:

Food and nutrition program for pregnant women, infants and children up to age five who meet income guidelines and are at medical or nutritional risk. www.health.state.mn.us/divs/fh/wic/

Breastfeeding Promotion:
Telephone and in-office support for breast feeding mothers. Promoting the health benefits of breast feeding to families, in the community and among health care providers.

Health Promotion: PartnerSHIP4Health:
Health teaching to provide information related to prevention or early detection of chronic health conditions.

Tobacco Use Prevention:
Tobacco compliance checks.



RESPONDS TO DISASTERS AND ASSISTS COMMUNITIES IN RECOVERY
Emergency Services:

Systems planning to assure a system is in place to address emergency services including individual emergencies and disasters.

ASSURES THE QUALITY AND ACCESSIBILITY OF HEALTH SERVICES
Family Planning:

Contract with local health care providers and the Community Action Council to obtain family planning health exams and prescriptions.

HELP ME GROW: parentsknow.state.mn.us
Case finding for children at risk for developmental delays. Case management with Interagency Committees to assess, plan and implement services for children with special needs.

Child and Teen Outreach:
Outreach to families on MA and Minnesota Care about the need for obtaining well child exams and dental checkups.

School Health:
Hearing and vision screening programs for the school age child. School nurse and school health aids in Battle Lake and Pelican Rapids schools.

Long Term Care Consultation :
Identify community resources to enable persons to remain in their own homes or to authorize their admission to a nursing home. Minnesota Senior Health Options care coordination.

Personal Attendant Authorization:
Assess the need, develop a plan and authorize personal care attendant services for persons with a chronic disabling condition.

Senior Services:
Information and referral services to individuals and families about the services available for the elderly. Health education programs based upon the wishes of senior groups. Advocacy to create and maintain a system of care delivery that addresses the needs of the elderly. Medicare parte D drug coverage.
