An absentee ballot for a Federal, State or Countywide election can be obtained by downloading an absentee ballot application from the following site:http://www.sos.state.mn.us/index.aspx?page=211

Once completed the signed application can be either mailed or faxed to the following:

If you are unable to download an absentee ballot application then a signed written request containing the following information is acceptable:

Your name,
The address of your legal residence,
Your phone number,
The address where you want your absentee ballot mailed,
The reason you are requesting an absentee ballot, and
For which election you are requesting the absentee ballot.

Remember to sign your letter requesting an absentee ballot and if you are requesting an absentee ballot for more than one individual (for example - a spouse) using the same letter then all of the individuals wishing to receive an absentee ballot must sign that letter.
